 Referrals
Anyone is welcome to make an application to our programs. If you know of a loved one, a friend or relative, a neighbour who might benefit from Day Centre Service, ask them if you can share their contact information with us. A site Coordinator will contact them to share information on our program and referral process.
If an individual receives service through the local Community Care Access Centre, the Case Manager can forward needed information to the Day Centre with your
approval.
A visit to the site is most welcome and can be reassuring to the individual. Contact the Day Centre closest to you or call the Community Care Access Centre in your county.
